[patch] Parallel Howto

This patch adds a "parallel howto" chapter to the tutorial.

I've put the generated HTML and PDF for the tutorial on the internal 
downloads:

There are several FIXME's still in the patch.  I want to add performance 
results to compare and contrast the various approaches.  Most of these 
boil down to running existing u-benchmark cases.  I will fix these 
before committing.
