[patch] Parallel version of Scalable SAR application

The attached patch modifies the SSAR application to run in parallel.  
Note that a temporary version of fftshift() that works correctly with 
distributed views has been added to kernel1.hpp.   A more efficient 
method of dealing with that is in planning.

To run, make sure LAM is started and type 'make parallel'.  This runs 
the parallel equivalent of 'make check'.
