[patch] tutorial updates for parallel chapter

This patch:
  - applies Mark's 7/31 edits
  - updates the existing example programs to match the text
  - adds missing example programs for parallel local views and
    parallel foreach
  - makes some misc fixes in the library

Next step is to separate out the serial fast convolution section to be 
the first chapter.

Patch applied.
