Can you hold off on applying this patch?  I need to look at the 
specification to see how we should handle a comparison between Index<1> 
and index_type.  I suspect that this ma may be related to the 
replacement of operator==(Index<1>, Index<1>) with template 
<dimensino_type Dim> operator==(Index<dim>, Index<dim).
