[patch] Fix lvalue_proxy issue (#51), Use test_assert

This patch adds operator= specializations for Lvalue_proxy.  This closes 
issue #51.  regr_proxy_lvalue_conv.cpp should now start to pass (in 
fact, all of our tests should pass now!).

Also, this patch changes most of the tests to use test_assert instead of 
assert.  This makes the test results meaningful when compiling with 
regular assertions turned off.
