Proposed modification for the size of the attachments upload form

I've attached a patch file that corrects an issue that has annoyed us
for a while: when using the gui on Windows (either IE or Firefox), the
window that opens up with the attachment upload form is too small for
the form.

This patch is a one-liner that increases the window size just enough to
allow all the controls to be visible. I've not tested it on other
platforms, but it should be safe.
