PATCH: Generalize build_doc command

I've been working on some documentation updates and fighting, once 
again, with the fact that everybody seems to put DocBook in different 
places.  This patch generalizes the build_doc command, so that (a) 
Debian Sarge is handled correctly, and (b) it's possible to override the 
location of the SGML declaration of XML, the DocBook DTD, and the DSSL 
stylesheets.  I've also made it possible to turn on and off the various 
kinds of documentation we generate (HTML, PDF, and internal).
